drm/atomic: Clear crtc_state->active in drm_atomic_helper_set_config.

This fixes some regressions in i915 when converting to atomic.
set_config failed with -EINVAL, and I received the following warning
in dmesg:

[drm:drm_atomic_crtc_check] [CRTC:20] active without enabled

Solve this by clearing active when a crtc is disabled.

Because crtc_state->enable implies that connectors are active the
change from disabled->enabled can only happen for the crtc that's
being set_config'd, and checking for !crtc_state->enable is sufficient
here.

One more round of drm-misc, again mostly atomic. Big thing is the
userspace blob code from Daniel Stone, with support for the mode_id blob
now added to the atomic ioctl. Finally we can do atomic modesets!

Note that the atomic ioctl is still behind the module knob since the
weston patches aren't quite ready yet imo - they lack TEST_ONLY support,
which is a fairly crucial bit of the atomic api. But besides that I think
it's all good to go. That's also why we didn't bother to hide the new blob
ioctls behind the knob, that part won't need to change. And if weston
patches get in shape in time we could throw the "atomic by default patch"
on top for 4.2.

  - register/unregister a process as a debugged process
  - Set address watch-point in the debugged process's GPU kernel

  The debugged process can only perform debug operation on itself. It is
  blocked by the amdkfd+H/W from performing operations on other processes's
  waves or GPU kernels. The blocking is done by setting the VMID and PASID of
  the debugged process in the packets that are sent to the CP with the debug
  instructions.

- Add support for static user-mode queues. These queues are regular queues,
  but because they belong to the debugged process, we need to make sure the CP
  doesn't preempt them during a debug operation. Therefore, we mark them as
  static for the CP ignore them during preemption.

- Support killing all the waves when a process is terminated. This is needed
  in case a process is terminated but we can't UNMAP its queues (can occur due
  to several reasons). In that case, the CP could be stuck unless we kill all
  its waves. This function is *very* important as it provides the kernel a high
  level of control over the GPU. The reason we didn't upstream this function
  so far, is because it is implemented using the H/W debugger module functions,
  so we had to wait until we can upstream the H/W debugger module.

- Replace declaration of bitmap from unsigned long to standard DECLARE_BITMAP

Add virtio gpu driver.

This patch adds a kms driver for the virtio gpu.  The xorg modesetting
driver can handle the device just fine, the framebuffer for fbcon is
there too.

Qemu patches for the host side are under review currently.

The pci version of the device comes in two variants: with and without
vga compatibility.  The former has a extra memory bar for the vga
framebuffer, the later is a pure virtio device.  The only concern for
this driver is that in the virtio-vga case we have to kick out the
firmware framebuffer.

Initial revision has only 2d support, 3d (virgl) support requires
some more work on the qemu side and will be added later.

This commit makes sure that on process termination, after
we're destroying all the active queues, we're killing all the
existing wave front of the current process.

By doing this we're making sure that if any of the CUs were blocked
by infinite loop we're enforcing it to end the shader explicitly.

This patch adds the skeleton H/W debugger module support. This code
enables registration and unregistration of a single HSA process at a
time.

The module saves the process's pasid and use it to verify that only the
registered process is allowed to execute debugger operations through the
kernel driver.

This patch adds support for static user-mode queues in QCM.
Queues which are designated as static can NOT be preempted by
the CP microcode when it is executing its scheduling algorithm.

This is needed for supporting the debugger feature, because we
can't allow the CP to preempt queues which are currently being debugged.

The number of queues that can be designated as static is limited by the
number of HQDs (Hardware Queue Descriptors).

drm/amdkfd: add H/W debugger IOCTL set definitions

This patch adds four new IOCTLs to amdkfd. These IOCTLs expose a H/W
debugger functionality to the userspace.

The IOCTLs are:

- AMDKFD_IOC_DBG_REGISTER:

The purpose of this IOCTL is to notify amdkfd that a process wants to use
GPU debugging facilities on itself only.
It is expected that this IOCTL would be called before any other H/W
debugger requests are sent to amdkfd and for each GPU where the H/W
debugging needs to be enabled. The use of this IOCTL ensures that only
one instance of a debugger is active in the system.

- AMDKFD_IOC_DBG_UNREGISTER:

This IOCTL detaches the debugger/debugged process from the H/W
Debug which was established by the AMDKFD_IOC_DBG_REGISTER IOCTL.

- AMDKFD_IOC_DBG_ADDRESS_WATCH:

This IOCTL allows to set different watchpoints with various conditions as
indicated by the IOCTL's arguments. The available number of watchpoints
is retrieved from topology. This operation is confined to the current
debugged process, which was registered through AMDKFD_IOC_DBG_REGISTER.

- AMDKFD_IOC_DBG_WAVE_CONTROL:

This IOCTL allows to control a wavefront as indicated by the IOCTL's
arguments. For example, you can halt/resume or kill either a
single wavefront or a set of wavefronts. This operation is confined to
the current debugged process, which was registered through
AMDKFD_IOC_DBG_REGISTER.

Because the arguments for the address watch IOCTL and wave control IOCTL
are dynamic, meaning that they could vary in size, the userspace passes a
pointer to a structure (in userspace) that contains the value of the
arguments. The kernel driver is responsible to parse this structure and
validate its contents.

dma_alloc_coherent() can return memory in the vmalloc range.
virt_to_page() cannot handle such addresses and crashes. This
patch detects such cases and obtains the struct page * using
vmalloc_to_page() instead.

After much discussion, give up on only passing a subset of SCSI commands
to userspace and pass them all. Based on what pscsi is doing, make sure
to set SCF_SCSI_DATA_CDB for I/O ops, and define attributes identical to
pscsi.

Make hw_block_size configurable via dev param.

Remove mention of command filtering from tcmu-design.txt.

There is just one configfs subsystem in the target code, so we might as
well add two helpers to reference / unreference it from the core code
instead of passing pointers to it around.

This fixes a regression introduced for v4.1-rc1 with commit 9ac8928e6,
where configfs_depend_item() callers using se_tpg_tfo->tf_subsys would
fail, because the assignment from the original target_core_subsystem[]
is no longer happening at target_register_template() time.

Correct a regression introduced with 8453eebd [MIPS: Fix strnlen_user()
return value in case of overlong strings.] causing assembler warnings
and broken code generated in __strnlen_kernel_nocheck_asm:

arch/mips/lib/strnlen_user.S: Assembler messages:
arch/mips/lib/strnlen_user.S:64: Warning: Macro instruction expanded into multiple instructions in a branch delay slot

with the CPU_DADDI_WORKAROUNDS option set, resulting in the function
looping indefinitely upon mounting NFS root.

Use conditional assembly to avoid a microMIPS code size regression.
Using $at unconditionally would cause such a regression as there are no
16-bit instruction encodings available for ALU operations using this
register.  Using $v1 unconditionally would produce short microMIPS
encodings, but would prevent this register from being used across calls
to this function.

The extra LI operation introduced is free, replacing a NOP originally
scheduled into the delay slot of the branch that follows.

dm: fix casting bug in dm_merge_bvec()

dm_merge_bvec() was originally added in f6fccb ("dm: introduce
merge_bvec_fn").  In that commit a value in sectors is converted to
bytes using << 9, and then assigned to an int.  This code made
assumptions about the value of BIO_MAX_SECTORS.

A later commit 148e51 ("dm: improve documentation and code clarity in
dm_merge_bvec") was meant to have no functional change but it removed
the use of BIO_MAX_SECTORS in favor of using queue_max_sectors().  At
this point the cast from sector_t to int resulted in a zero value.  The
fallout being dm_merge_bvec() would only allow a single page to be added
to a bio.

This interim fix is minimal for the benefit of stable@ because the more
comprehensive cleanup of passing a sector_t to all DM targets' merge
function will impact quite a few DM targets.

dm-multipath accepts 0 path mapping.

  # echo '0 2097152 multipath 0 0 0 0' | dmsetup create newdev

Such a mapping can be used to release underlying devices while still
holding requests in its queue until working paths come back.

However, once the multipath device is created over blk-mq devices,
it rejects reloading of 0 path mapping:

  # echo '0 2097152 multipath 0 0 1 1 queue-length 0 1 1 /dev/sda 1' \
      | dmsetup create mpath1
  # echo '0 2097152 multipath 0 0 0 0' | dmsetup load mpath1
  device-mapper: reload ioctl on mpath1 failed: Invalid argument
  Command failed

With following kernel message:
  device-mapper: ioctl: can't change device type after initial table load.

DM tries to inherit the current table type using dm_table_set_type()
but it doesn't work as expected because of unnecessary check about
whether the target type is hybrid or not.

Hybrid type is for targets that work as either request-based or bio-based
and not required for blk-mq or non blk-mq checking.

When stacking request-based dm device on non blk-mq device and
device-mapper target could not map the request (error target is used,
multipath target with all paths down, etc), the WARN_ON_ONCE() in
free_rq_clone() will trigger when it shouldn't.

The warning was added by commit aa6df8d ("dm: fix free_rq_clone() NULL
pointer when requeueing unmapped request").  But free_rq_clone() with
clone->q == NULL is valid usage for the case where
dm_kill_unmapped_request() initiates request cleanup.

Fix this false warning by just removing the WARN_ON -- it only generated
false positives and was never useful in catching the intended case
(completing clone request not being mapped e.g. clone->q being NULL).

Since commit 100832abf065bc18 ("usb: isp1760: Make HCD support
optional"), CONFIG_USB_ISP1760_HCD is automatically selected when
needed.  Enabling that option in the defconfig is now a no-op, and no
longer enables ISP1760 HCD support.

Re-enable the ISP1760 driver in the defconfig by enabling
USB_ISP1760_HOST_ROLE instead.

Without this frontbuffer flip when enabling planes PSR got compromised
and wasn't being enabled waiting forever on the flush that never
arrived.

Another solution would to create a enable_cursor function and split this
frontbuffer flip among the different plane enable and disable functions.
But if necessary this can be done in a follow up work. For now let's
just fix the regression.

The commit [49fb18972581: ALSA: hda - Set stream_pm ops automatically
by generic parser] resulted in regressions on some Realtek and VIA
codecs because these drivers set patch_ops after calling the generic
parser, thus stream_pm got cleared to NULL again.  I haven't noticed
since I tested with IDT codec.

Restore (partial revert) the stream_pm ops for them to fix the
regression.

d_walk() might skip too much

when we find that a child has died while we'd been trying to ascend,
we should go into the first live sibling itself, rather than its sibling.

Off-by-one in question had been introduced in "deal with deadlock in
d_walk()" and the fix needs to be backported to all branches this one
has been backported to.

of the bitmap and handle the one-filling the remainder of the last
partial bitmap block.  The loop conditional expects count to be signed
in order to detect when the final block is processed, after which count
goes negative.

Unfortunately, a recent change made this unsigned along with some other
related fields.  The result of is this is that during mount,
omfs_get_imap will overrun the bitmap array and corrupt memory unless
number of blocks happens to be a multiple of 8 * blocksize.

Fix by changing count back to signed: it is guaranteed to fit in an s32
without overflow due to an enforced limit on the number of blocks in the
filesystem.

A static checker found the following issue in the error path for
omfs_fill_super:

    warn: missing error code here? 'd_make_root()' failed. 'ret' = '0'

Fix by returning -ENOMEM in this case.

fs, omfs: add NULL terminator in the end up the token list

match_token() expects a NULL terminator at the end of the token list so
that it would know where to stop.  Not having one causes it to overrun
to invalid memory.

In practice, passing a mount option that omfs didn't recognize would
sometimes panic the system.
