PM / OPP: Documentation: Fix opp-microvolt in examples
The triplet present in "opp-microvolt" property should be in the order
<target min max>, while all the examples have it in the order
<min target max>.
Fix it.
Luckily all of the users of "opp-microvolt" property have applied brain
instead of copying the examples from documentation and none of the
actual dts files have it wrong.

illegal memory access.
The bug was detected using KASAN framework.

PM / OPP: Expose _of_get_opp_desc_node as dev_pm_opp API
Rename _of_get_opp_desc_node to dev_pm_opp_of_get_opp_desc_node and add it
to include/linux/pm_opp.h to allow other drivers, such as platform OPP
and cpufreq drivers, to make use of it.

PM / OPP: Move away from RCU locking
The RCU locking isn't well suited for the OPP core. The RCU locking fits
better for reader heavy stuff, while the OPP core have at max one or two
readers only at a time.
Over that, it was getting very confusing the way RCU locking was used
with the OPP core. The individual OPPs are mostly well handled, i.e. for
an update a new structure was created and then that replaced the older
one. But the OPP tables were updated directly all the time from various
parts of the core. Though they were mostly used from within RCU locked
region, they didn't had much to do with RCU and were governed by the
mutex instead.
And that mixed with the 'opp_table_lock' has made the core even more
confusing.
Now that we are already managing the OPPs and the OPP tables with kernel
reference infrastructure, we can get rid of RCU locking completely and
simplify the code a lot.
Remove all RCU references from code and comments.
Acquire opp_table->lock while parsing the list of OPPs though.

Take reference of the OPP table from within _find_opp_table(). Also
update the callers of _find_opp_table() to call
dev_pm_opp_put_opp_table() after they have used the OPP table.
Note that _find_opp_table() increments the reference under the
opp_table_lock.
Now that the OPP table wouldn't get freed until the callers of
_find_opp_table() call dev_pm_opp_put_opp_table(), there is no need to
take the opp_table_lock or rcu_read_lock() around it. Drop them.

PM / OPP: Update OPP users to put reference
This patch updates dev_pm_opp_find_freq_*() routines to get a reference
to the OPPs returned by them.
Also updates the users of dev_pm_opp_find_freq_*() routines to call
dev_pm_opp_put() after they are done using the OPPs.
As it is guaranteed the that OPPs wouldn't get freed while being used,
the RCU read side locking present with the users isn't required anymore.
Drop it as well.
This patch also updates all users of devfreq_recommended_opp() which was
returning an OPP received from the OPP core.
Note that some of the OPP core routines have gained
rcu_read_{lock|unlock}() calls, as those still use RCU specific APIs
within them.

Now that we have proper kernel reference infrastructure in place for OPP
tables, use it to guarantee that the OPP table isn't freed while being
used by the callers of dev_pm_opp_set_*() APIs.
Make them all return the pointer to the OPP table after taking its
reference and put the reference back with dev_pm_opp_put_*() APIs.
Now that the OPP table wouldn't get freed while these routines are
executing after dev_pm_opp_get_opp_table() is called, there is no need
to take opp_table_lock. Drop them as well.
Remove the rcu specific comments from these routines as they aren't
relevant anymore.
Note that prototypes of dev_pm_opp_{set|put}_regulators() were already
updated by another patch.

PM / OPP: Add 'struct kref' to OPP table
Add kref to struct opp_table for easier accounting of the OPP table.
Note that the new routine dev_pm_opp_get_opp_table() takes the reference
from under the opp_table_lock, which guarantees that the OPP table
doesn't get freed unless dev_pm_opp_put_opp_table() is called for the
OPP table.
Two separate release mechanisms are added: locked and unlocked. In
unlocked version the routines aren't required to take/drop
opp_table_lock as the callers have already done that. This is required
to avoid breaking git bisect, otherwise we may get lockdeps between
commits. Once all the users of OPP table are updated the unlocked
version shall be removed.

PM / OPP: Don't allocate OPP table from _opp_allocate()
There is no point in trying to find/allocate the table for every OPP
that is added for a device. It would be far more efficient to allocate
the table only once and pass its pointer to the routines that add the
OPP entry.
Locking is removed from _opp_add_static_v2() and _opp_add_v1() now as
the callers call them with that lock already held.
Call to _remove_opp_table() routine is also removed from _opp_free()
now, as opp_table isn't allocated from within _opp_allocate(). This is
handled by the routines which created the OPP table in the first place.

PM / OPP: Add light weight _opp_free() routine
The OPPs which are never successfully added using _opp_add() are not
required to be freed with the _opp_remove() routine, as a simple kfree()
is enough for them.
Introduce a new light weight routine _opp_free(), which will do that.
That also helps us removing the 'notify' parameter to _opp_remove(),
which isn't required anymore.
Note that _opp_free() contains a call to _remove_opp_table() as the OPP
table might have been added for this very OPP only. The
_remove_opp_table() routine returns quickly if there are more OPPs in
the table. This will be simplified in later patches though.

PM / OPP: Fix memory leak while adding duplicate OPPs
There are two types of duplicate OPPs that get different behavior from
the core:
A) An earlier OPP is marked 'available' and has same freq/voltages as
the new one.
B) An earlier OPP with same frequency, but is marked 'unavailable' OR
doesn't have same voltages as the new one.
The OPP core returns 0 for the first one, but -EEXIST for the second.
While the OPP core returns 0 for the first case, its callers don't free
the newly allocated OPP structure which isn't used anymore. Fix that by
returning -EBUSY instead of 0, but make the callers return 0 eventually.
As this isn't a critical fix, its not getting marked for stable kernel.

KVM: s390: do not expose random data via facility bitmap
kvm_s390_get_machine() populates the facility bitmap by copying bytes
from the host results that are stored in a 256 byte array in the prefix
page. The KVM code does use the size of the target buffer (2k), thus
copying and exposing unrelated kernel memory (mostly machine check
related logout data).
Let's use the size of the source buffer instead. This is ok, as the
target buffer will always be greater or equal than the source buffer as
the KVM internal buffers (and thus S390_ARCH_FAC_LIST_SIZE_BYTE) cover
the maximum possible size that is allowed by STFLE, which is 256
doublewords. All structures are zero allocated so we can leave bytes
256-2047 unchanged.
Add a similar fix for kvm_arch_init_vm().

xhci: remove WARN_ON if dma mask is not set for platform devices
The warn on is a bit too much, we will anyway set the dma mask if not set
previously.
The main reason for this fix is that 4.10-rc1 has a dwc3 change that
pass a parent sysdev dev pointer instead of setting the dma mask of
its xhci platform device. xhci platform driver can then get more
attributes from the sysdev than just the dma mask.
The usb core and xhci changes are not yet in 4.10, and a fix like
this was preferred instead of taking those big changes this late in
the rc-cycle.

powerpc: Ignore reserved field in DCSR and PVR reads and writes
IBM bit 31 (for the rest of us - bit 0) is a reserved field in the
instruction definition of mtspr and mfspr. Hardware is encouraged to
(and does) ignore it.
As a result, if userspace executes an mtspr DSCR with the reserved bit
set, we get a DSCR facility unavailable exception. The kernel fails to
match against the expected value/mask, and we silently return to
userspace to try and re-execute the same mtspr DSCR instruction. We
loop forever until the process is killed.
We should do something here, and it seems mirroring what hardware does
is the better option vs killing the process. While here, relax the
matching of mfspr PVR too.

ARM: dts: da850-evm: fix read access to SPI flash
Read access to the SPI flash are broken on da850-evm, i.e. the data
read is not what is actually programmed on the flash.
According to the datasheet for the M25P64 part present on the da850-evm,
if the SPI frequency is higher than 20MHz then the READ command is not
usable anymore and only the FAST_READ command can be used to read data.
This commit specifies in the DTS that we should use FAST_READ command
instead of the READ command.

virtio/s390: support READ_STATUS command for virtio-ccw
As virtio-1 introduced the possibility of the device manipulating the
status byte, revision 2 of the virtio-ccw transport introduced a means
of getting the status byte from the device via READ_STATUS. Let's wire
it up for revisions >= 2 and fall back to returning the stored status
byte if not supported.

tools/virtio/ringtest: fix run-on-all.sh for offline cpus
Since
ef1b144d ("tools/virtio/ringtest: fix run-on-all.sh to work
without /dev/cpu") run-on-all.sh uses seq 0 $HOST_AFFINITY as the list
of ids of the CPUs to run the command on (assuming ids of online CPUs
are consecutive and start from 0), where $HOST_AFFINITY is the highest
CPU id in the system previously determined using lscpu. This can fail
on systems with offline CPUs.
Instead let's use lscpu to determine the list of online CPUs.

virtio_console: fix a crash in config_work_handler
Using control_work instead of config_work as the 3rd argument to
container_of results in an invalid portdev pointer. Indeed, the work
structure is initialized as below:
INIT_WORK(&portdev->config_work, &config_work_handler);
It leads to a crash when portdev->vdev is dereferenced later. This
bug
is triggered when the guest uses a virtio-console without multiport
feature and receives a config_changed virtio interrupt.

This is to silence an uninitialized variable warning in debug output.
The problem is this line:

If "head == vq->num" is true on the first iteration then "out" and "in"

nbd: only set MSG_MORE when we have more to send
A user noticed that write performance was horrible over loopback and we
traced it to an inversion of when we need to set MSG_MORE. It should be
set when we have more bvec's to send, not when we are on the last bvec.
This patch made the test go from 20 iops to 78k iops.

arm64: avoid returning from bad_mode
Generally, taking an unexpected exception should be a fatal event, and
bad_mode is intended to cater for this. However, it should be possible
to contain unexpected synchronous exceptions from EL0 without bringing
the kernel down, by sending a SIGILL to the task.
We tried to apply this approach in commit
9955ac47f4ba1c95 ("arm64:
don't kill the kernel on a bad esr from el0"), by sending a signal for
any bad_mode call resulting from an EL0 exception.
However, this also applies to other unexpected exceptions, such as
SError and FIQ. The entry paths for these exceptions branch to bad_mode
without configuring the link register, and have no kernel_exit. Thus, if
we take one of these exceptions from EL0, bad_mode will eventually
return to the original user link register value.
This patch fixes this by introducing a new bad_el0_sync handler to cater
for the recoverable case, and restoring bad_mode to its original state,
whereby it calls panic() and never returns. The recoverable case
branches to bad_el0_sync with a bl, and returns to userspace via the
usual ret_to_user mechanism.

HID: wacom: Fix sibling detection regression
Commit
345857b ("HID: wacom: generic: Add support for sensor offsets") included
a change to the operation and location of the call to 'wacom_add_shared_data'
in 'wacom_parse_and_register'. The modifications included moving it higher up
so that it would occur before the call to 'wacom_retrieve_hid_descriptor'. This
was done to prevent a crash that would have occured when the report containing
tablet offsets was fed into the driver with 'wacom_hid_report_raw_event'
(specifically: the various 'wacom_wac_*_report' functions were written with the
assumption that they would only be called once tablet setup had completed;
'wacom_wac_pen_report' in particular dereferences 'shared' which wasn't yet
allocated).
Moving the call to 'wacom_add_shared_data' effectively prevented the crash but
also broke the sibiling detection code which assumes that the HID descriptor
has been read and the various device_type flags set.
To fix this situation, we restore the original 'wacom_add_shared_data'
operation and location and instead implement an alternative change that can
also prevent the crash. Specifically, we notice that the report functions
mentioned above expect to be called only for input reports. By adding a check,
we can prevent feature reports (such as the offset report) from
causing trouble.

ARCv2: IOC: Adhere to progamming model guidelines to avoid DMA corruption
On AXS103 release bitfiles, DMA data corruptions were seen because IOC
setup was not following the recommended way in documentation.
Flipping IOC on when caches are enabled or coherency transactions are in
flight, might cause some of the memory operations to not observe
coherency as expected.
So strictly follow the programming model recommendations as documented
in comment header above arc_ioc_setup()

arm64/ptrace: Reject attempts to set incomplete hardware breakpoint fields
We cannot preserve partial fields for hardware breakpoints, because
the values written by userspace to the hardware breakpoint
registers can't subsequently be recovered intact from the hardware.
So, just reject attempts to write incomplete fields with -EINVAL.

arm64/ptrace: Avoid uninitialised struct padding in fpr_set()
This patch adds an explicit __reserved[] field to user_fpsimd_state
to replace what was previously unnamed padding.
This ensures that data in this region are propagated across

libceph: make sure ceph_aes_crypt() IV is aligned
... otherwise the crypto stack will align it for us with a GFP_ATOMIC
allocation and a memcpy() -- see skcipher_walk_first().

x86/ioapic: Restore IO-APIC irq_chip retrigger callback
commit
d32932d02e18 removed the irq_retrigger callback from the IO-APIC
chip and did not add it to the new IO-APIC-IR irq chip.
Unfortunately the software resend fallback is not enabled on X86, so edge
interrupts which are received during the lazy disabled state of the
interrupt line are not retriggered and therefor lost.
Restore the callbacks.

ovl: fix possible use after free on redirect dir lookup
ovl_lookup_layer() iterates on path elements of d->name.name
but also frees and allocates a new pointer for d->name.name.
For the case of lookup in upper layer, the initial d->name.name
pointer is stable (dentry->d_name), but for lower layers, the
initial d->name.name can be d->redirect, which can be freed during
iteration.

Keep the count of remaining characters in the redirect path and calculate
the current position from that. This works becuase only the prefix is
modified, the ending always stays the same.

arm64: mm: avoid name clash in __page_to_voff()
The arm64 __page_to_voff() macro takes a parameter called 'page', and
also refers to 'struct page'. Thus, if the value passed in is not
called 'page', we'll refer to the wrong struct name (which might not
exist).

cpu/hotplug: Remove unused but set variable in _cpu_down()
After the recent removal of the hotplug notifiers the variable 'hasdied' in
_cpu_down() is set but no longer read, leading to the following GCC warning
when building with 'make W=1':
kernel/cpu.c:767:7: warning: variable ‘hasdied’ set but not used [-Wunused-but-set-variable]
Fix it by removing the variable.

powerpc/perf: Use MSR to report privilege level on P9 DD1
SIER and SIAR are not updated correctly for some samples, so force the
use of MSR and regs->nip instead for misc_flag updates. This is done by
adding a new ppmu flag and updating the use_siar logic in
perf_read_regs() to use it, and dropping the PPMU_HAS_SIER flag.
